You now have the option to complete your New Patient forms online. In order to use this quick and convenient option you will need a security code. If you did not receive a security code on your welcome letter, please call the office at (260) 490-4673 to receive your personal code. This process is confidential and secure and will transmit your information directly into your personal electronic chart within our office.

If you are unable to complete the information online, you may also print the paperwork, complete it, and bring with you to your appointment. If neither of these options are convenient for you, please arrive to your appointment 15-20 minutes early to complete the paperwork.
